TRY LIGHT PROGRAM

PH offers a Try Light Program allowing Participant to rent photobiomodulation equipment for at-home use, including transcranial photobiomodulation devices manufactured by Vielight and LED photobiomodulation devices manufactured by Celluma (collectively, the "Equipment"). The Program is offered solely for personal exploration and general wellness purposes and is not a medical service.

The initial Try Light period consists of a two-month rental for a total fee of $300, payable in full prior to receipt of the Equipment and non-refundable. At the conclusion of the two-month period, Participant may (a) return the Equipment, (b) purchase the Equipment outright, or (c) continue renting under the rent-to-own option. Purchase prices are $2,200 for Vielight devices and $1,550 for Celluma devices. The rent-to-own option is $200 per month, automatically billed every thirty (30) days, with all continuation payments applying toward the applicable purchase price. Ownership transfers only upon PH's receipt of full payment of the purchase price.

Participant authorizes PH to automatically charge the payment method provided for all fees due under this Agreement, including rental payments, unpaid balances, replacement costs, late fees, and any other amounts owed. Participant agrees to maintain a valid payment method on file at all times while in possession of the Equipment. If a payment is declined, returned, or disputed, Participant remains responsible for the full amount due. PH may immediately suspend the rental and require return of the Equipment upon payment failure. Participant agrees to reimburse PH for any reasonable collection costs, including administrative fees, collection agency fees, court costs, and reasonable attorney's fees incurred in recovering unpaid amounts.

All Equipment remains the sole property of PH until ownership transfers. Participant assumes full responsibility for the Equipment while in Participant's possession, including responsibility for loss, theft, damage, misuse, or failure to return the Equipment. Participant agrees to pay the full replacement cost if the Equipment is lost, stolen, or damaged beyond normal wear and tear. Replacement cost shall equal the applicable purchase price. PH is authorized to charge the payment method on file for any unpaid balance or replacement cost.

Participant may permit other individuals to use the Equipment, provided that Participant remains fully and solely responsible for the Equipment at all times. Participant assumes all responsibility and liability for any loss, damage, misuse, injury, claim, or expense arising from use of the Equipment by any third party, whether authorized or unauthorized.

Participant agrees to use the Equipment only as instructed, not to modify, disassemble, repair, or alter the Equipment, and not to use the Equipment in any manner inconsistent with provided guidance. PH reserves the right to terminate this Agreement and require immediate return of the Equipment in the event of nonpayment, misuse, or violation of these terms.

Participant acknowledges and agrees that PH does not provide medical advice, diagnosis, or treatment; does not diagnose, treat, cure, or prevent any disease or condition; and does not guarantee any outcome or result. The Equipment is not a substitute for professional medical care. Participant is solely responsible for consulting with a licensed healthcare provider regarding any health concerns, conditions, or contraindications.

Participant acknowledges that use of light-based wellness devices may involve inherent risks, including light sensitivity or unexpected physiological or psychological responses. Participant voluntarily and knowingly assumes all risks associated with use of the Equipment, whether known or unknown, foreseeable or unforeseeable.

To the fullest extent permitted by law, Participant releases, waives, and holds harmless Project Healthspan, LLC and its owners, members, employees, contractors, and affiliates from any and all claims, demands, damages, losses, liabilities, or causes of action arising out of or related to use or misuse of the Equipment, including claims arising from ordinary negligence.

Participant agrees to indemnify, defend, and hold harmless PH from and against any claims, damages, losses, liabilities, costs, or expenses arising from Participant's use of the Equipment or from use by any third party to whom Participant permitted access.

PH may terminate this Agreement immediately upon Participant's failure to comply with its terms. Upon termination or demand, Participant shall immediately return the Equipment in good condition, normal wear and tear excepted. Failure to return Equipment upon demand constitutes unauthorized retention and Participant remains liable for the full replacement cost.

This Agreement may be executed electronically and accepted through electronic means, including electronic signature or clicking "I agree." Participant agrees that such electronic acceptance constitutes a legally binding signature, that this Agreement is enforceable as a written contract, and that electronic records satisfy any legal requirement for a writing or signature. A copy of this Agreement in electronic form shall be deemed an original.

This Agreement shall be governed by and construed in accordance with the laws of the State of Oregon, without regard to its conflict-of-laws principles.
